Questionnaire About You
Before your first visit with us, we will send you some questionnaires via SMS. These questionnaires include questions about different aspects of your life situation and your mental health. The reason we ask you to complete these in advance is so that your therapist will already have some knowledge about you when you meet. By answering these questions beforehand, you’ll be able to spend more time during the session talking about the issues that are most important to you.
The answers you provide in the questionnaires can also be used to monitor how you are doing throughout the course of your treatment. This is done by having you answer some of the same questions at multiple points in time.
The questionnaires will only be read by your therapist and other healthcare professionals who are involved in your care. We recommend that you answer as openly and honestly as you can.

Your Rights as a Patient
From the moment you are informed that you have been granted the right to healthcare and will receive an appointment with us, you have the status of a patient. As a patient, there are certain rights that it is important for you to be aware of. Below you will find an overview of the most relevant rights for you, along with links to more information.
Your Rights in Case of a Breach of Deadline
When you are granted the right to healthcare, meaning you are informed that you will receive an appointment at the hospital, you also have the right for the healthcare to be initiated within a specified deadline. This deadline is provided to you along with the notification that you have been granted the right to healthcare. If the hospital fails to provide you with an appointment within this deadline, it is called a breach of deadline. In case of a breach, you have the right to choose between receiving healthcare elsewhere or waiting until the hospital can offer you an appointment. Right to User Participation
As a patient, you have the right to contribute to the design of the healthcare you receive. You have the right to receive information about various treatment options and to participate in the decision about what kind of treatment you should receive. Complaint and Compensation Rights
If you believe that you have not received the healthcare you are entitled to or have been subjected to malpractice, you have the right to file a complaint. Who you should complain to depends on the nature of the complaint. Sometimes, you should first complain to the treatment facility, while other times, you can complain directly to the supervisory authorities. If you sustain an injury directly as a consequence of the treatment you have received, you can apply for compensation through the Norwegian Patient Injury Compensation (Norsk pasientskadeerstatning). Right to Access Your Patient Journal
All treatment at Norwegian hospitals must be continuously documented in a patient journal. The journal records the treatment you have received, as well as assessments and conclusions from the healthcare professionals who meet and treat you. You have the right to read what is written about you in your patient journal. You can access this by logging in to Helsenorge.no.
